Register as a student at PHAME


Thank you for your interest in the PHAME Academy program. To enroll your student into our program please download the following forms and carefully follow the directions. No student will be admitted into PHAME without completed forms.

Code of Conduct (Adobe PDF)
This form lays out the expectations of all PHAME students and is designed to promote a safe and fun learning environment.
Please read through all expectations carefully before signing and dating.
This form is required for admittance into PHAME.

Student Information Form (Adobe PDF)
This form provides us with information about the student enrolling in the PHAME program.
Please be sure to include all relevant contact information, doctor and prescription information, and other student needs that the PHAME staff needs to be aware of.
Form must be signed and dated before submitting. This form is required for admittance into PHAME.

Please check the classes that your student is wanting to participate in.
Next, check which payment plan you will be using.
If you are in need of tuition assistance to help pay for part of or all of your student's tuition this term, you will need to write a letter to the Board of Directors declaring your request for tuition assistance, how much you plan to pay for, and how much in tuition assistance you are requesting.

You will be contacted regarding tuition assistance approval shortly after all appropriate materials are submitted.
The enrollment form is required for admittance into PHAME.
